Physician/Patient relationship must be established prior to treatment. Consent to treat must be given in writing prior to treatment.

PRP treatments: PRP treatments purchased as part of the Black Friday Special can be used for orthopedic or cosmetic conditions. The PRP treatment is for up to 3 orthopedic areas. The PRP treatments can be used anytime before December 31, 2025. PRP treatments can be transferred or gifted. PRP treatments can be purchased by existing patients or new patients. New Patients will be required to have an orthopedic evaluation at an additional charge. If an existing patient is using the PRP treatment for an orthopedic/musculoskeletal injury that has not been previously evaluated or treated by Dr. Logan, the patient will be required to have an orthopedic evaluation at an additional charge.
Stem Cell: StemCell Treatments are Adipose Derived Autologous cells that are minimally manipulated.  The cells are harvested from the patient via a small volume fat extraction. StemCell Treatments expire December 31,2025. StemCell Treatments can be transferred or gifted. StemCell treatments can be purchased by existing patients or new patients. New Patients will be required to have an orthopedic evaluation at an additional charge. If an existing patient is using the StemCell treatment for an orthopedic/musculoskeletal injury that has not been previously evaluated or treated by Dr. Logan, the patient will be required to have an orthopedic evaluation at an additional charge.
